Vagus Nerve Stimulation for Hiccups: Does It Actually Work?

Vagus nerve stimulation has helped some people with severe, treatment-resistant hiccups, but current evidence is limited to a small number of case reports and the response is not predictable. Most reported successes involve surgically implanted cervical VNS in people whose hiccups continued for months or years despite other treatments. At the same time, some cases describe little benefit, and VNS itself has also been reported to trigger persistent hiccups.

The reason is that the vagus nerve is involved in the hiccup reflex, but it is only one part of a larger network. Electrical stimulation does not simply “switch hiccups off.” VNS is not a standard treatment for ordinary hiccups, and persistent hiccups require evaluation for an underlying cause before invasive stimulation would even be considered.

Does vagus nerve stimulation work for hiccups and what the clinical evidence suggests

What Role Does the Vagus Nerve Play in Hiccups?

The vagus nerve contributes sensory input to the hiccup reflex, but it does not control the reflex by itself. A simplified pathway looks like this:

Trigger → vagal, phrenic and sympathetic sensory input → central processing in the brainstem and spinal pathways → phrenic motor output → diaphragm contraction → glottic closure → hiccup.

This matters because hiccups are sometimes described online as simply a “vagus nerve problem.” That explanation is too simple. The phrenic nerve provides the main motor pathway to the diaphragm, while vagal and other sensory pathways contribute signals that help organize the reflex. Current research supports a distributed reflex network rather than one single proven “hiccup center.”

Why Might VNS Help?

If persistent hiccups involve abnormal sensory input or repetitive central processing, changing vagal signals could theoretically interrupt that cycle. Researchers have proposed that VNS may alter afferent signaling or influence how the hiccup reflex is processed in the central nervous system.

This provides a plausible explanation for why some refractory cases improved after VNS. However, the mechanism has not been established, and hiccups should not be interpreted simply as a condition caused by “too little” or “malfunctioning” vagal activity.

Does Vagus Nerve Stimulation Stop Intractable Hiccups?

Role of the vagus nerve in the hiccup reflex pathway and diaphragm contraction

Sometimes, according to published case reports—but the evidence is far too limited to consider VNS a proven hiccup treatment. The most important finding from the literature is not a success rate, but the range of outcomes: some patients improved completely, some improved partially, some did not respond, and some developed hiccups after stimulation.

Published Evidence Outcome What It Suggests
Early implanted VNS case Complete resolution of chronic intractable hiccups Shows that VNS can work in selected refractory cases
Reported treatment failure No meaningful improvement despite months of stimulation adjustments Response is not predictable
Refractory case report Major but incomplete improvement Benefit does not always mean complete resolution
2024 implanted VNS case Complete resolution after stimulation settings were adjusted Suggests settings may influence individual response
VNS-related adverse case Persistent hiccups appeared after stimulation and improved after stopping VNS Stimulation can also provoke the reflex

What the Evidence Supports

Implanted VNS may help a very small subset of people with severe, treatment-resistant hiccups. Published cases show that the approach is biologically plausible and can produce substantial improvement in some individuals who have not responded to other treatments.

However, these reports describe exceptional cases rather than a predictable treatment response. They show that VNS can work, not that it works reliably.

What the Evidence Does Not Support

Current evidence does not provide a reliable success rate or identify who is most likely to benefit. The available studies are limited by very small sample sizes, different underlying causes of hiccups, different stimulation settings and the lack of randomized controlled trials.

Case-report evidence is also vulnerable to publication bias because unusual positive or negative outcomes may be more likely to be reported. A few successful cases should therefore not be converted into a percentage chance that VNS will stop hiccups.

Why Can Vagus Nerve Stimulation Sometimes Cause Hiccups?

Because the vagus nerve is part of the hiccup reflex pathway, stimulation can influence that circuit in more than one direction. It may interrupt the repetitive activity involved in some cases, but it may also become an additional trigger in others.

A published case of VNS-related hiccups in a person with epilepsy showed that severe persistent hiccups developed after stimulation began and eventually stopped after the device was permanently deactivated. This type of report is important because it shows that vagal stimulation is not automatically anti-hiccup; the same pathway involved in the reflex can potentially be influenced in different ways.

The response may depend on factors such as stimulation intensity, frequency, pulse width, electrode location, individual anatomy and the condition causing the hiccups. A 2024 refractory-hiccup case also suggested that stimulation settings may influence outcomes, as symptoms improved after adjustments were made.

The key lesson is not that VNS “fails” or “causes” hiccups universally. It is that the vagus nerve participates in a complex reflex network rather than acting like a simple switch. More stimulation does not automatically mean fewer hiccups.

When Would VNS Ever Be Considered for Hiccups?

Vagus nerve stimulation case reports for severe and treatment-resistant hiccups

VNS has only been considered in exceptional situations: when hiccups are prolonged, disabling and resistant to more conventional approaches. A short episode after eating or drinking is a completely different situation from months of treatment-resistant hiccups affecting sleep, nutrition or daily function.

Situation Typical Next Step
Short-lived hiccups Usually resolve without medical treatment
Hiccups lasting more than 48 hours Evaluate for possible underlying causes
An identifiable cause is found Treat the underlying condition when possible
Persistent symptoms Medication or targeted treatments may be considered
Severe refractory hiccups Specialist procedures may enter discussion
Exceptional treatment-resistant cases Implanted VNS has occasionally been attempted

Hiccups lasting longer than 48 hours are generally considered persistent, while episodes lasting beyond about a month are commonly described as intractable. Before considering invasive procedures, clinicians usually focus on identifying and treating possible causes and may consider medications such as baclofen, chlorpromazine or metoclopramide when appropriate.

Why Persistent Hiccups Need Evaluation

Persistent hiccups should not simply be treated as a “vagus nerve problem.” They can be associated with many different factors, including central nervous system conditions, gastrointestinal or thoracic problems, metabolic abnormalities, infection, medication effects or postoperative changes.

The practical difference is important: a brief episode after a meal is usually a temporary reflex event, while hiccups that continue for days and interfere with eating, sleeping or breathing may signal a problem that needs medical evaluation.

What About Non-Invasive or Ear-Based VNS for Hiccups?

There is currently not enough evidence to consider non-invasive cervical VNS or auricular VNS an established treatment for persistent or intractable hiccups. Most published hiccup cases involve implanted cervical VNS, and even those reports remain limited to individual cases rather than controlled clinical trials.

Approach Current Evidence for Hiccups
Implanted cervical VNS Several refractory-hiccup case reports with complete, partial and unsuccessful responses
Transcutaneous cervical/vagal stimulation Very limited clinical evidence
Auricular VNS / taVNS Very early human case-level evidence; recent positive reports involved combined treatment
Consumer ear-based VNS devices No established evidence for treating persistent or intractable hiccups

Auricular VNS has recently appeared in human persistent-hiccup research, but the evidence is still very early. In a 2026 case report, hiccups resolved after percutaneous auricular VNS was combined with repeated ultrasound-guided phrenic nerve blocks. Because both treatments were used together, the case cannot determine how much benefit came from auricular stimulation itself.

Non-invasive auricular taVNS research for hiccups and early human evidence

For ear-based stimulation, ZenoWell Luna Plus is an example of a non-invasive auricular taVNS wellness device. It uses stimulation through the outer ear, which is a different approach from the implanted cervical VNS systems described in most refractory-hiccup reports. Because stimulation location, parameters and device design differ across approaches, evidence from implanted VNS cannot simply be transferred to consumer ear-based devices. Current research on auricular stimulation remains exploratory and does not establish Luna Plus or other consumer taVNS devices as a hiccup treatment.

Vagus Nerve and Hiccups FAQs

Can Stimulating the Vagus Nerve Stop Hiccups?

Possibly in rare, highly refractory cases, but the evidence is based mainly on case reports. Published outcomes include complete responses, partial improvements and treatment failures, so electrical VNS is not considered a routine treatment for ordinary hiccups.

How Is the Vagus Nerve Involved in Hiccups?

The vagus nerve contributes sensory input to the hiccup reflex along with the phrenic nerve and sympathetic pathways. Central brainstem and spinal networks then coordinate the signals that lead to diaphragm contraction and glottic closure.

Can Vagus Nerve Stimulation Make Hiccups Worse?

Yes. Persistent hiccups have been reported after implanted VNS, and in some cases changing or stopping stimulation altered the symptoms. This is one reason VNS should not be viewed as a simple anti-hiccup switch.

Is VNS Approved for Hiccups?

VNS is not an established standard indication for hiccups. Published reports describe rare experimental or compassionate use in severe refractory cases rather than a routine approved treatment pathway.

Can I Stimulate My Vagus Nerve at Home to Stop Hiccups?

Breathing exercises, humming, cold exposure, massage and consumer electrical devices are not equivalent to the implanted VNS protocols described in refractory-hiccup case reports. If hiccups last more than 48 hours, continuing to try stronger “vagus nerve hacks” should not replace medical evaluation.

When Should Hiccups Be Checked by a Doctor?

Hiccups lasting more than 48 hours should be evaluated, especially if they interfere with eating, sleeping or breathing, cause significant discomfort or occur with other concerning symptoms. At that point, identifying the underlying cause is usually more important than trying additional home remedies.
